In consideration of the acceptance of the Registrant’s registration with MelRad Racing, the Registrant and/or their Parent/Guardian agree as follows:

1. As a Registrant of MelRad Racing, I (we) are required to adhere to Triathlon Canada’s/USAT’s policies and procedures (including its Code of Conduct and Discipline) as well as the World Anti-Doping Code. I (we) further understand that MelRad Racing respects that Triathlon Canada/USAT is a member of the International Triathlon Union and is governed by its rules and regulations.
2. To adhere to MelRad Racing’s policies and procedures;
3. If MelRad Racing does not have a signed waiver or assumption of risk form on file, then the Registrant will not be permitted to participate in events or training with MelRad Racing.
4. That the Registrant is aware of, and agrees to abide by, the bylaws, policies, rules and regulations of Triathlon Canada and USAT.
5. They have read, understand, comprehend and executed the Waiver or Assumption of Risk form, as applicable
6. To accept sole responsibility for the Registrant’s personal possessions and equipment.
7. That I (we) have read and agree to be bound by the Triathlon Canada or USAT Code of Conduct and Ethics and applicable disciplinary procedures and may be subject to disciplinary action accordingly for prohibited conduct, acts or practices.
8. That I (we) agree to adhere to and abide by the World Anti-Doping Program.
9. I (we) have reviewed this form and execution indicates my/our agreement with everything described herein.
10. I (we) acknowledge that I (we) have read this form in its entirety and that I (we) have executed this form voluntarily.
11. The Participant and/or their Parent/Guardian acknowledge that they have read this Registration Form in its entirety and that I have executed this Registration Form voluntarily.

1. This is a binding legal agreement; therefore clarify any questions or concerns before signing.

As a Participant in the activities, programs and events of my applicable sanctioned Triathlon Club MelRad Racing, Coaching Program MelRad Multisport, and MelRad Racing Activities, and the sport of triathlon (collectively the “Activities”), the undersigned acknowledges and agrees to the following terms:

2. Melanie McQuaid and MelRad Racing, organizers, members, employees, coaches, volunteers, officials, participants, agents, sponsors, owners/operators of applicable facilities, and representatives (the “Organizations”) are not responsible for any injury, property damage, expense, loss of income, damage or loss of any kind suffered by a Participant during, or as a result of, the Activities, caused in any manner whatsoever including, but not limited to, the negligence of the Organizations.

Description of Risks

3. I am participating voluntarily in the Activities. In consideration of my participation, I hereby acknowledge that I am aware of the risks, dangers and hazards associated with or related to the Activities. The risks, dangers and hazards include, but are not limited to, injuries from:

1. Swimming, biking (mountain biking, road cycling and cross riding) and running;
2. Executing strenuous and demanding physical techniques;
3. Vigorous physical exertion, strenuous cardiovascular workouts and rapid movements;
4. Exerting and stretching various muscle groups;
5. Entering the water by either diving or jumping;
6. Extended time in water and underwater;
7. Extreme weather and temperature conditions which may result in dehydration, heatstroke, sunstroke or hypothermia;
8. Mounting, dismounting or falling off a bicycle;
9. Falls to the ground due to uneven or irregular terrain or surfaces;
10. Failure to properly use any piece of equipment or from the mechanical failure of any piece of equipment;
11. Contact, colliding, falling or being struck by equipment, other participants, or equipment;
12. Collisions with fences, poles, stands, and other equipment;
13. Extreme weather conditions which may result in heatstroke, sunstroke, or hypothermia;
14. Spinal cord injuries which may render me permanently paralyzed; and
15. Travel to and from events which are an integral part of the Events.

RELEASE OF LIABILITY

4. In consideration of the MelRad Racing allowing me to participate, I agree:

1. That my physical condition has been verified by a medical doctor to participate in the Activities;
2. The sole responsibility for my safety remains with me;
3. To remove myself if I sense or observe any unusual hazard or unsafe condition, or feel unable or unfit to safely continue;
4. To ASSUME all risks arising out of, associated with or related to my participation;
5. To WAIVE any and all claims that I may have now or in the future against the Organizations;
6. To freely ACCEPT AND FULLY ASSUME all such risks and possibility of personal injury, death, property damage, expense and related loss, including loss of income, resulting from my participation in the activities, events and programs of the Organization; and
7. I agree to waive any and all claims and to release Melanie McQuaid, employees, representatives and contractors (hereinafter “the Releasees”) from any and all liability for any loss, damages (including direct, indirect, special and/or consequential), expense, costs (including legal fees), or injury, including death, that I or my next of kin may suffer as a result of training for the sport of triathlon or mountain biking or my use of or presence at MelRad Multisport Coaching services, due to any cause whatsoever, including negligence, negligent rescue, omissions, carelessness, breach of contract or breach of any statutory or other duty of care, on the part of the Releasees. I understand that negligence includes failure on the part of the Releasees to take reasonable steps to safeguard or protect me from or warn me of the risks, dangers and hazards of training for triathlon, racing endurance sports, or participating in mountain biking.

5. I acknowledge that I have read this agreement and understand it, that I have executed this agreement voluntarily, and that this agreement is to be binding upon myself, my heirs, spouse, children, parents, guardians, next of kin, executors, administrators and legal or personal representatives.

6. By signing or typing in your name below and ticking/checking the “I Agree” icon, you agree to be bound this Legal Agreement.

Our mandate is to provide a world class coaching environment and experience. For us to deliver this we only work with a limited number of athletes at each camp. Upon acceptance of your application we gave you priority and subsequently had to turn away other applicants.  However, we understand that due to extenuating circumstances you may have to cancel your registration.  In light of the challenge organizing these camps you agree to the following:

• Camp fees are non-refundable
• Fees can be transferred to a future camp minus $100 administration fee
• You can transfer your registration to another athlete if you organize this 14 days in advance.
